a)facility -  where employees were allowed to use a Dayton 1 ton hoist to
lift miscellaneous
types of welding equipment weighing approximately 300 pounds and the hook
on the hoist was
missing a latch hook, the hoist had not been inspected in the past 12
months, the upper limit
switch had not been tested.
