A.  Employer knew or should have known that employee operating Lull

544B, was exposed to the hazard of being struck by forklift in the event
of rollover at Main
Street Methodist Church job site.  A feasible and useful method to correct
this hazard is to
ensure operator always wears his seat belt when operating the forklift.
