A.  Employer knew or should have known that employees driving a TCM,

event of a rollover.  A feasible and useful method of abatement, among

an operable seat belt and enforce its use.
B.  Employer knew or should have known that employees working on, near, or
around steam
pipes on boiler were exposed to the hazard of being burned.  A feasible
and useful method of
abatement, among others, is to cover and or insulate the exposed pipes on
the boiler in the
maintenance shop.
