A.  Employer knew or should have known that one forklift operator

forklift, Number 735, without a seat belt, was exposed to the hazard of
falling out of the cab
during an accident.  A feasible and useful method to correct the hazard,
among others, is to:
1) Install a seat belt on the forklift; and 2) Ensure that employees use
the seat belt while
operating the forklift.
