the hazard of falling from, or being pinned or crushed underneath,
forklift.  A feasible and
useful method to correct this hazard, among other methods, is to require
operator to wear set
belt when operating Mitsubishi forklift, Equipment Number Wilift-15, no
serial number,
located at plant.
