A.  Employer knew or should have known that operator not wearing seat belt
on Gradall
forklift, Model 524, moving steel material at Cedar Creek Church site was
exposed to the
hazard of being thrown from and struck by forklift in the event of
rollover.  A feasible and
useful method to correct this hazard is to ensure operator wears seat belt
while driving
forklift.
