40.1-51.1(a) Code of Virginia:   The employer, City of Waynesboro (Public
Works Shops), did not furnish a place
of employment which was free from recognized hazards that were causing or
likely to cause death or serious physical
harm to employees in that hoists are operated by employees in the Mechanic
shop that have not been regularly
inspected according to nationally recognized standards, exposing them to
injury should the hoists fail.
In the Public Works shops, two hoists are used by employees [5- ton Wright
and a 5- ton Budget hand chain operated
hoists].   These two hoists have not been inspected, monthly or annually,
nor has any record been kept of those
inspections.
Note: American National Standards Institute (ANSI) B30.16- 1987 calls for
monthly inspections of all controls and
operating mechanisms safety devices, hooks, chains, and chain stretch;
annual inspections of loose bolts, excessive
wear of chain, chain stretch, hook, supporting structure, and end
connections of load chains.  Inspections conducted
must be documented, dated, and signed to indicate required inspection have
been completed for items inspected
inspected during an annual inspection.
Among other methods, one feasible and acceptable method to correct this
hazards is to conduct the inspections called
for by ANSI B30.16- 1987, American national Standard Safety Code for
Hoists.
