were likely to cause death or serious
harm to employee in that employees were exposed to crushing hazards:
A.  Maintenance Bay:  At the time of the inspection safety locks were not
in place on the 2 Benwil swing
arm lifts.
B.  Maintenance Bay:  At the time of the inspection employees operating
the 2 Benwil swing arm lifts had
not been properly trained in their safe operation.
One feasible and useful method to correct the hazard, among others is to:
1.  Have a competent person/company thoroughly inspect the two lifts and
perform needed maintenance
and repairs.
2.Develop, implement and document a program of regular inspections per the
lift owners manual.
3.Have a competent person/company train the employees on safe operation of
the lifts.
