Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of ammonia inhalation. a) On or about January 19, 2005: Plant Engine Room was not provided with tight-fitting doors. (Reference Source: ANSI/ASHRAE 15-1978, Section 11.13.5a). b) On or about January 19, 2005: Garage Engine Room was not provided with either a continuously operated, independent mechanical ventilation system or electrical equipment which conforms to either the requirements for a Class 1, Division 2 Location as defined in the National Electrical Code (Reference Source: ANSI/ASHRAE 15-1978, Section 7.4.1 et al). Among other methods, feasible and acceptable abatement methods to correct the hazards are to: 1.Repair the existing doors so they are tight fitting for the Plant Engine Room as required by ANSI/ASHRAE 15-1978, Section 11.13.5a. 2.Install a continuously operated, independent mechanical system in the Garage Engine Room as required in Reference Source: ANSI/ASHRAE 15-1978, Section 7.4.1 et al. 3.Install electrical equipment which conforms to either the requirements for a Class 1 Division 2 location as defined in the National Electrical Code as required in Class 1, Division 2 Location as defined in the National Electrical Code ANSI/ASHRAE 15-1978, Section 7.4.1 et al. ABATEMENT CERTIFICATION REQUIRED
