(a) Glacier Precast, Inc.; The 5T Nitchi lacked records for critical items
such as hoisting
machinery, sheaves, hooks, chains, ropes, and other lifting devices;
(b) Glacier Precast, Inc.; The 3T Jet lacked records for critical items
such as hoisting
machinery, sheaves, hooks, chains, ropes, and other lifting devices;
(c) Glacier Precast, Inc.; satellite location; The 5T Robbins and Myers
lacked records for critical
items such as hoisting machinery, sheaves, hooks, chains, ropes, and other
lifting devices;
(d) Glacier Precast, Inc.; The 5T Nitchi underhung hoist was not equipped
with an operable
latch;
(e) Glacier Precast, Inc.; The 3T Nitchi underhung hoist was not equipped
with an operable
latch;
(f) Glacier Precast, Inc.; The 5T Robbins and Myers underhung hoist was
not equipped with and
operable latch; and
(g) Glacier Precast, Inc.; satellite location; The spreader bar used to
lift fabricated products with
the underhung hoists was not an approved lifting device.
Feasible means to abate include preparation of required inspection
documentations, repair of
broken or inoperable latches as described in B30.17-1998 OVERHEAD AND
GANTRY
CRANES (TOP RUNNING BRIDGE, SINGLE GIRDER, UNDERHUNG HOIST); provide and
use approved below-the-hook lifting devices when executing lifts with
underhung hoists as as
referenced in B30.17-1998 OVERHEAD AND GANTRY CRANES (TOP RUNNING BRIDGE,
SINGLE GIRDER, UNDERHUNG HOIST) and described in B30.20 Below-the-Hook
Lifting
Devices.  B30.17-1998 OVERHEAD AND GANTRY CRANES (TOP RUNNING BRIDGE,
SINGLE GIRDER, UNDERHUNG HOIST) references ASME B30.16 discussing the
elements
of a chain hoist inspection.

angle indicator;
(b) Glacier Precast, Inc.; The 15T DeVault truck crane lacked the required
angle indicator;
(c) Glacier Precast, Inc.; The 15T DeVault truck crane was not equipped
with an operable latch
on the hook accembly; and
(d) Glacier Precast, Inc.; The live line on the load assembly of the 15T
DeVault truck crane had
a wire rope clip attached to it above the wedge socket.
Feasbile methods of abatement include the replacement of latches on load
hooks, equip truck
cranes with angle indicators and re-rig the line on the 15T DeVault such
that all abatement
conforms to the specification listed in B30.5-1994 for Mobil and
Locomotive Cranes.ive
