that were causing or likely to cause death or serious physical harm to

ground below while
being elevated on the forks of a Terex TH1056C Turbo rough terrain

of a safety platform and without the operator being within 25 feet of the
operators seat.

correct this hazard
would be to provide equipment designed for personnel lifting, e.g. scissor
lift or articulated
boom lift or follow the requirements of ASME B56.6-1992 and ASME
B56.6a-1994
