Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a struck by hazard:
On or about December 27, 2004, worn parts and/or parts substituted for
manufacturer approved replacement parts were used on BJ slip-type elevator
spider assembly in use on Rig Number 183.
Among methods of corrections, feasible and acceptable abatement methods to
correct this hazard includes complying with manufacturer's requirement in
the BJ
Hughes Operating Service and Parts Manual and the International
Association of
Drilling Contractors' Drilling Manual, 1992 edition, in the design and
implementation of a comprehensive inspection  program for all hoisting
tools,
including elevator and slip assemblies.  This program would include, but
not be
limited to:
a.A combination of field daily inspections, monthly  inspections,
quarterly
inspections and disassembly inspections.
b.Inspections and maintenance repairs would be made only by technically
competent personnel.
c.Written records of all inspection and maintenance procedures would be
maintained.
d.Only approved replacement parts would be used to make repairs.ent
