(A)Job Site: On July 19, 2005, and at times prior thereto, employees

Clark and Toyota powered industrial trucks, were not required to wear seat
belts.
ABATEMENT NOTE:  Amongst other methods, one feasible and acceptable method
of
abatement to correct this hazard is to require the use of seat belts when
operating forklifts
(powered industrial trucks) throughout the establishment as required by
ANSI/AMSE B56.1.
