Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a)At the drilling site, on or about 08/17/2005: Employees performing maintenance on the mast of a water well drill rig were permitted to ride the rotary head to the top of the mast because the employer failed to effectively prohibit the practice. ABATEMENT: One feasible means of abatement would be to prohibit and enforce the practice of using the rotary head as a personal lifting device in written safety/work instructions and ensure that employees use ladders and ladder safety devices. Note: Abatement certification and supporting documentation are required for this item.
