Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck by and crushing hazard: a)Natural gas pipeline construction site: On October 6, 2005, and at times prior thereto, a Fruehauf dolly and trailer were coupled to a Caterpillar 561N pipe layer tractor using a Caterpillar drawbar (pin). Secondary chains or cables were not used to secure the converter dolly and the loaded trailer to the tractor. The drawbar (pin) failed exposing employees to the hazard of being struck by and/or crushed by the overturning and sliding load. NOTE 1: A feasible and acceptable abatement method to correct this hazard is to use one or more safety devices (chain or cable) between the converter dolly and the tow vehicle. NOTE 2: Abatement documentation and certification are required for this item.
