(A)  The employer allowed an employee lift attachment which affected the
capacity and safe operation of a
Hyster 30 fork truck without the manufacturer's written approval.
(B)  The Hyster 30 fork truck was not marked to identify the attachment,
show the approximate weight of the
truck and attachment combination, and show the capacity of the truck with
attachment at maximum elevation
with load centered.
TOOL ROOM/MAINTENANCE:  THE HYSTER 30 FORK TRUCK, SER 8010B0303SF WAS USED
WITH A "HOMEMADE" EMPLOYEE LIFTING ATTACHMENT TO ACCESS CEILING LIGHTING
WITHOUT THE MANUFACTURER'S WRITTEN APPROVAL AND WAS NOT MARKED TO
IDENTIFY THE ATTACHMENT AND VARIOUS WEIGHT COMBINATIONS.S
