On or about 07/18/05, at the front yard of the residence, an employee was
exposed to a fall
hazard while using a field-modified fiberglass extension ladder.
Among other methods, one feasible and acceptable abatement method  to
correct this hazard is
to prohibit employees from using extension ladders that have been modified
without the approval
of the manufacturer.
PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS
VIOLATION.
