Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the inhalation of or contact with ammonia vapor due to
failure of the system
or system components including but not limited to piping, compressors and
pressure relief
devices, over pressurization of vessels and possible explosive
atmosphere(s) due to inadequate
installation, inspection, maintenance and/or utilization of the system and
its components:
Among other methods, feasible and acceptable abatement methods to correct
these hazards would
include the following:
1.Follow recognized and accepted good engineering practices according to
manufacturers
information and IIAR (International Institute of Ammonia Refrigeration)
Bulletin 110 pg.
3/39 and change out safety relief valves every five years and test the
relief valves after
removal.
2.Develop and implement standard operating procedures in accordance with
API (American
Petroleum Institute) Recommended Practice 750 Section 5 which contain the
following
information:  temporary operation conditions under which emergency
shutdown is
required, assignment of shutdown responsibility, operating limits,
consequences of
deviation, steps required to correct or avoid deviations, and precautions
necessary to
prevent exposure including but not limited to engineering controls,
administrative
controls and personal protective equipment.
3.Establish a system to promptly address the PHA teams findings and
recommendations in
accordance with API Recommended Practice 750 Section 3.
4.Implement management of change procedures in accordance with API
Recommended
Practice 750 Section 4.
5.Place a prominent sign having letters sufficiently large to be easily
read
at the king valve
in accordance with IIAR Bulletin 109 Section 4.10.3.
6.Develop and implement written procedures to ensure the mechanical
integrity of the
ammonia refrigeration system in accordance with API Recommended Practice
750
Section 8, such as but not limited to the ammonia sensor, emergency
shutdown buttons
and testing of safety cutouts on vessels and compressors.
