Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being pulled into the rotating parts of a
pipe threading machine:
a).  First Floor Level, Main Entrance Area.  On or about 1/6/05, an
employee operated
a Ridgid 300 Pipe Machine that was not equipped with a momentary contact
device as
required by American National Standard for Portable Pipe Threading
Machines (ANSI)
B208.1982, Section 4.4.  The machine was activated by a toggle switch that
would
not
automatically shut off if the operator was inadvertently caught up in the
rotating parts and
2 inch diameter stock.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to install a properly guarded momentary contact device (constant
pressure foot
switch).
