Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a fall or burn hazard associated with an improperly
installed auxiliary escape
system:
On February 23, 2005 and at other times previously thereto, the auxiliary
escape
line was installed in an unsafe manner and included the following visible
deficiencies:
a.The Crosby clips used to create an eye were installed incorrectly by
placing the saddle on the dead end of the wire rope;
b.The Crosby clips used were not evenly spaced with an adequate length of
dead-end wire rope.  Only two Crosby clips were used instead of the
recommended minimum of three and these clips were badly corroded;
c.The ground anchor for the auxiliary escape line was incorrectly
installed.
The employer permitted the use of a hammered metal stake as the ground
anchor.  The stake was hammered into the ground without a buried
counter weight, nor was it augured in with a twist anchor as recommended
by the manufacturer.
Among other methods, one feasible and acceptable abatement method involves
the
employer ensuring the emergency escape line system is installed in
accordance
with Section 6.10 of the American Petroleum Institute's Recommended
Practice
54 (3rd Edition; 1999), as well as Crosby clip and Geronimo company
manufacturer guidelines.

were exposed to struck-by hazards associated with sudden pressure release:
On February 23, 2005, and at other times prior thereto, employees were
exposed
to struck-by hazards involved with sudden pressure release in that the
employer
did not:
a.ensure the installation of safety-catch chains for union joint
connections
of high pressure mud hoses and staked out lines;
b.regularly conduct and document the inspection of critical mechanical
parts
(threaded and otherwise) including but not limited to the mud pump
pressure relief valve.
Among other methods, one feasible and acceptable abatement method to
correct
the hazard is to initiate and maintain an inspection program for the high
pressure
equipment associated with the mud pumps.  Equipment must be installed and
maintained in good operating condition in accordance with of the American
Petroleum Institute's Recommended Practice 54 (Third Edition, August 1999).

were exposed to the hazard of being struck by falling equipment or debris
in the event of a rig
collapse:
On February 23, 2004, and at other times prior thereto, at this well site,
the
employer failed to follow good engineering practices in the guying and
anchoring
of the drilling rig derrick structure prior to commencing operations:
a.Open hooks of undetermined capacity on chain termination points were
used to wrap around and secure the guy lines to steel base plates;
b.The base plates onto which the hooks were connected was kept in place
by two hammered-in stakes with mushroomed heads;
c.There were no records maintained on the anchorage system verifying the
anchorage system had been tested within the previous two (2) years.
Among other methods, one feasible and acceptable abatement method to
correct
the hazard is to follow guidelines described in the American Petroleum
Institute's
(API) Recommended Practice 54 (August, 1999) and API 4-G (First Edition,
January 1992).
