Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential crush injuries from a falling load with an inadequately engineered lifting devices: (a)ASF-Keystone - Employees used homemade below-the-hook lifting devices that had no permanently affixed durable identification stating the rated capacity, manufacturer, serial number, lifting device weight, etc. and did not receive all required inspections, on or about March 4, 2005. Among other methods, one feasible and acceptable abatement method to correct this hazard includes all below the hook devices shall have the rated capacity, manufacturerer, serial number, lifting device weight as outlined in the American Society of Mechanical Engineers standard for Below-the-Hook Lifting Devices (ASME B30.20 - 1993). Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential striking injuries from a drifting load: (a)ASF-Keystone - Employees used a Wright one ton hoist and the boom drifts if not tied to a piece of equipment, on or about March 4, 2005. Among other methods, one feasible and acceptable abatement method to correct this hazard includes repairing the hoist/boom, as outlined in the American National Standard Institute standard for Monorail Systems and Underhung Cranes (ANSI B30.11 - 1973). VERIFICATION REQUIRED
