Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling loads: (a)Plantwide - Load Ratings/Maximum Lifting Capacities had not been established for in-house constructed under hook lifting devices including but not limited to roof load bar and assemblies, floor load bar and assemblies, sidewall hooks, sidewall clamps, partition wall hooks and OSB lift frames, on or about June 23, 2006. (b)Plantwide - An inspection program had not been established for in-house constructed under hook lifting devices including but not limited to roof load bar and assemblies, floor load bar and assemblies, sidewall hooks, sidewall clamps, partition wall hooks and OSB lift frames, on or about June 23, 2006. (c)Plantwide - In-house constructed under hook lifting devices including but not limited to roof load bar and assemblies, floor load bar and assemblies, sidewall hooks, sidewall clamps, partition wall hooks, and OSB lift frames had not been marked with load ratings and identification information, on or about June 23, 2006. ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to construct and use these lifting devices in accordance with the provisions of Chapter 20-1, of the Standard for Below-the Hook Lifting Devices - ASME B30.20 - 2003. VERIFICATION REQUIRED

ABATEMENT NOTE: Among other methods, one feasible and acceptable method to correct these hazards is to establish lifting capacities in accordance with the hoist manufacturer's operations manual and in accordance with ANSI B30.11 - Standard for Monorail Systems and Underhung Cranes - Chapter 11-2.2 1999 Addenda. VERIFICATION REQUIREDTION

(b)Ceiling Finishing Area - Employee was required to work under a suspended roof assembly weighing up to 8,400 pounds, on or about June 23, 2006. ABATEMENT NOTE: Among other methods, one feasible and acceptable method to abate this hazard is to develop work practices and operational procedures that prohibit employees from working under the load in accordance with the Standard for Below-the Hook Lifting Devices - ASME B30.20 - 2003, ANSI B30.11 - Standard for Monorail Systems and Underhung Cranes and ANSI B30.16 - Standard for Overhead Hoists. VERIFICATION REQUIRED
