Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to severe burn injuries from a potential dust explosion
hazard that could have
resulted from, or been exacerbated by the (a) failure to install
appropriate dust collection
system(s); b) failure to locate the Grizzly dust collector(s) that
collects combustible/explosive
dust outside of the building; c)  failure to locate the Hammond dust
collector that collects
combustible/explosive dust outside the building; d)  failure to install
appropriate isolation devices
between pieces of equipment connected by ductwork; (e)  failure to install
isolation devices from
the dust collector upstream to the work area(s) and (f)  failure to
enclose interior stairs in a
dusttight shaft with a self closing type or automatic closing type door:
(a)Basement, Finishing Areas 1 and 2 - An appropriate type of dust
collection
system was not installed in the area to capture combustible dust generated
from
hand sanding operations, on or about March 9, 2006.
(b)Basement, Grizzly Dust Collector/Sanding Area - The Grizzly Model G9980
dust
collector used to collect combustible dust from the Grizzly fifty one (51)
inch
wide belt sander was not located outside, on or about March 9, 2006.
(c)First Floor, Hammond Dust Collector - The Hammond Dust Collector, model
DK
1255 used to collect combustible dust from equipment in the tooling
department
(including the double headed disc sander) was not located outside, on or
about
March 9, 2006.
(d)First Floor, Hammond Dust Collector - The ductwork connecting the
various
pieces of woodworking and the double headed disc sander was not provided
with
appropriate isolation devices, such as chokes, rotary valves, fast acting
valves,
flame front diverters, or flame front extinguishing system(s), on or about
March
9, 2006.
(e)First Floor - Hammond Dust Collector - Isolation device(s) were not
provided to
prevent deflagration propagation from the dust collector upstream to the
work
area (tooling room), on or about March 9, 2006.
(f)Interior Stairs - The rear set of stairs leading from the basement to
the tooling
room was not enclosed in a dust-tight shaft that was provided with a self
closing
type or automatic closing type of door, on or about March 9, 2006.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable abatement
method to correct these hazards (a) through (f) is to comply with National
Fire Protection
Association (NFPA) 654 "Standard for the Prevention of FIre and Dust
Explosions from
the Manufacturing, Processing, and Handling of Combustible Particulate
Solids".  This
includes the following:
(a)Install an appropriate type of dust collection system per Section 4.1
of the NFPA
654.
(b)Relocate or install an appropriate type of dust collector outside of
the building per
Section 3.13.1.1 of NFPA 654.  As an alternative to relocation of the
collector
comply with the exceptions listed in the NFPA 654.
(c)Relocate or install an appropriate type of dust collector outside of
the building per
Section 3.13.1.1 of NFPA 654.  As an alternative to relocation of the
collector
comply with the exceptions listed in the NFPA 654.
(d)Install appropriate isolation devices per Section 3.1.3 of the NFPA 654.
(e)Install appropriate isolation devices per Section 3.1.3.2 of the NFPA
654.
(f)Install appropriate stairs and doors per Section 2.3.8 of the NFPA 654.
