Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential crushing injuries from falling loads while using an inadequately engineered lifting device and chains not made of steel alloy: (a)Main Shop - Employees used a below the hook lifting device to lift grates. The lifting device was not marked with the rated capacity, manufacturer, serial number, and lifting device weight. The device was not inspected periodically or proof tested and certified, on or about January 19, 2007. (b)Main Shop - Employees used a below the hook lifting device and non steel alloy chains to lift grates. The lifting device was not marked with the rated capacity, manufacturer, serial number and lifting device weight. The device was not inspected periodically or proof tested and certified. The chains used for lifting were not steal alloy, on or about January 19,2007. (c)Robot Shop - Employees used a below the hook lifting device to lift grates. The lifting device was not marked with the rated capacity, manufacturer, serial number and lifting device weight. The device was not inspected periodically or proof tested and certified, on or about January 19, 2007. (d)Robot Shop - Employees used a below the hook lifting device to lift grates. The lifting device was not marked with the rated capacity, manufacturer, serial number and lifting device weight. The device was not inspected periodically or proof tested and certified, on or about January 19,2007. (e)Dip Shed, Large Tank - Employees used a below the hook lifting device to lift grates. The lifting device was not marked with the rated capacity, manufacturer, serial number and lifting device weight. The device was not inspected periodically or proof tested and certified, on or about January 19, 2007. (f)Dip Shed, Small Tank - Employees used a below the hook lifting device and non steel alloy chains to lift grates. The lifting device was not marked with the rated capacity, Manufacturer, serial number and lifting device weight. The device was not inspection periodically or proof tested and certified. The chains used for lifting were not steel alloy, on or about January 19, 2007. Among other methods, One feasible means of abatement would be to have a engineer determine the rated capacity and mark the information along with the manufacturer, serial number, lifting device weight on the device. The device must also be inspected periodically and proof tested and certified. Carbon steel chains must be replaced with steel alloy type. VERIFICATION REQUIRED
