40.1-51.1(a) of the Code of Virginia:  The employer, Byrd's Wrecking Yard,
Inc., did not furnish a place of
employment which was free from recognized hazards that were causing or
likely to cause death or serious physical
harm in that employees are required to operate chain hoist that had a
defective lifting chain attached exposing them to
injury from potential dropped load.
In the Shop a Yale 500 Pound Electric Chain Hoist had a roller chain that
had been wired together in several areas to
hold the roller chain together.    Wiring the chain together would not
provide the same lifting strength as would an
original manufactured chain rated for the loads being lifted.
One feasible and acceptable abatement method, among other methods, to
correct this hazard would have been to
replace the roller chain (and not wired it together) as required by ANSI
B30.16.2-1990, American National Standard
