were exposed to falls from elevation:
On April 20, 2005, and other times prior thereto, at this location,
employees
were departing a gangway by jumping from the gangway onto the shore.  The
gangway was not secured.  The gangway extended 10 feet over the shore and
was
raised 40 inches above the shore.
One means of abatement would be to provide a stairway or ladder at all
personnel
points of access where there is a break in elevation of 19 inches (48 cm)
or more,
and no ramp, runway, sloped embankment, or personnel hoist is provided.
