Sec. 18.60.075.  Safe Employment.
(a) An employer shall do everything necessary to protect the life, health,
and
safety of employees including, but not limited to:
(1) complying with all occupational safety and health standards and
regulations adopted by the department;
(2)furnishing and prescribing the use of suitable protective equipment,
safety
devices, and safeguards as are prescribed for the work and work place;
(3) adopting and prescribing control or technological procedures, and
monitoring and measuring employee exposure in connection with hazards, as
may
be necessary for the protection of employees; and
(4)  furnishing to each employee employment and a place of employment that
are
free from recognized hazards that, in the opinion of th4e commissioner, are
causing or are likely to cause death or serious physical harm to the
employees.
EXAMPLE:There were no written procedures for equipment operation and no
thorough training of sawmill equipment operators.  The safety officer said
that the saw operators knew how to take care of the machines but there is
no
documentation for training, and the manual for the Hamil gang-saw is
written
in Korean, and the manuals for the Scragg-saw and edger-saw are missing.
The assistant manager said that new employees were trained by the current
operators but without written procedures and documented operator training
by a
qualified trainer, there is no assurance that new operators know to observe
all operating procedures and safety practices with each piece of equipment.
An industrial accident occurred the GIO sawmill on July 24, 2006 where an
employee suffered an accident with an edger saw with the result that he had
two fingers and half of his right hand amputated.  The employee reached
into
the rear of the saw to clean out sawdust debris and made hand contact with
the
edger blades.  The employee was an occasional operator of the saw but was
not
familiar with all of the switches and control functions to safely operate
the
saw.  Employee saw operators had not received training and complete
operating
instructions to enable them to safely operate all of the saws in the
sawmill.
During the inspection six employees were interviewed and asked to
demonstrate
the E430 Edger Saw control switch operations to start and stop the saw.
The
Manager, Asst Manager, Safety Officer, the primary E430 saw operator, an
occasional E430 saw operator, and the LT300 mill saw operator were the
employees interviewed.  None of the six knew the functions of a rotary
switch
on the edger-saw electrical panel.  With the rotary switch in the "Hand"
position, the saw turned on when the electrical panel knife switch was
closed.
I asked them to turn off the saw with the emergency power switches and they
were not able to shut off the saw with either of the 2 emergency power
controls.  None of the six knew that when the rotary switch was turned to
the
"Hand" position that the emergency shut-off switches were bypassed.
Causal factors contributing to the accident include:
Inadequate employee training.
The employer had no program to ensure that all operators had been
effectively
trained to safely operate equipment.
The accident victim was not familiar with all edger saw operatingcontrols.
This employee had not been trained to recognize the potential hazard that
the
saw blades could still be in operation.
The employer failed to implement written procedures for the control of
hazardous energy.
Manuals for the E430 Edger-saw and Skragg-saw were not available for
employee
reference.
The Hamil gang-saw manual was written in Korean and the saw operators could
not read Korean.There were no written operating procedures for the E430
Edger-saw, Skragg-saw,
or Hanil gang-saw.
The employer failed to implement a safety program.  Prior to the 7/24/06
amputation accident, there was no safety officer, or safety meetings.
A rotary electrical switch installed on the Edger-saw electrical panel
bypassed the two operator emergency stop controls that shut down the saw.
As an apparent "Imminent Danger" hazard I told the manager that the machine
must not be used until the safe operation of the saw could be verified and
that operators were competent to operate the saw.  The manager agreed and a
lock was placed on the knife switch to lock out the saw and the main
breaker
powering the knife switch panel was turned off.
