employer did not furnish each of his employees conditions of employment

recognized hazards that were causing or likely to cause death or serious
physical harm to employees in that

a)establishment, alignment rack - where a Hunter RX Alignment Rack used a
metal wire clothes
hanger in lieu of a "C" clip to prevent safety locking bracket from coming
off shaft. The
employees were not trained to recognize the hazard or to remove defective
equipment out of
service.
Abatement note:
One feasible and acceptable abatement method, among others, to correct
this hazard is provide operator
training to recognize hazards, and to ensure that the lift is not used
until the repairs have been made.
