employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that:
a)  Lot #51, Pebblebrook Lane, Champaign, IL.  An employee was 8 feet
above grade AND
exposed to fall related hazards while working on a platform being raised
by a rough-terrain
forklift.  Fall protection was not utilized.
b)  Lot #51, Pebblebrook Lane, Chapaign, IL.  An employee was exposed to
caught-in and fall
related hazards while working on a platform being raised by a
rough-terrain forklift.  The
operator left the controls of the forklift.
Among feasible means of abatement would be to comply with American
National Standards
Institute (ANSI) B.56.1 - 1992, Safety Requirements for Rough-Terrain
Forklifts.
