were exposed to being struck by the heavy steel sides of concrete forms:
(a)  For the employee required to disassemble the metal box culvert forms
and exposed
to injury from the forms falling when not properly secured, located at
1101 158th Street
NE, Menoken, North Dakota.
Among other methods, one feasible and acceptable method to correct this
hazard is to
develop and implement a written procedure and to train employees in the
safe practices
for disassembly of concrete forms as identified in ANSI A10.9-1970,
American National
Standards Institute, Safety Requirements for Concrete Construction and
Masonry Work,
Section 6.4.5 which states, "Vertical forms being raised or removed in
sections
shall not
be released until adequately braced or secured".
