Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to an injury from a vertical lift
door which did not provide the door or door operator with a means for
connection of a reversing edge switch
or equivalent that will reverse the direction of the door upon striking an
object during the closing cycle.
a).Motor Pool, East side garage door:  The vertical lift door did not have
an  operational  reversing edge
switch which upon striking an object during the closing cycle would
reverse the direction of door.
b)Motor Pool, West side garage door: The vertical lift door did not have
an operational reversing edge
switch which upon striking an object would reverse the direction of the
door.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to repair the
reversing switch mechanism; or to place warning placards at the door
controls instructing the door operator to
ensure the doorway is cleared and to stand by the controls until the door
completes the closing cycle.
