Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by falling steel plates: On or about 5/30/06, the employer did not ensure that the defective plate screw clamps were not used during lifts to prevent employee exposure to the hazard of being struck by falling steel plates. a.West of torch machine; Renfroe screw clamp, MD. #SCPA, Serial #9616, with worn cam jaw and screw cup. b.Torch machine table; Renfroe screw clamp, MD. #SCPA, Serial #9808, with damaged and welded tightening screw. Among other methods, one feasible abatement method to correct these hazards is to ensure that lifting devices are subjected to frequent and periodic inspections, to repair or replace damaged and worn lifting devices, and provide employee training in the use of the devices, in accordance with the manufacturer's application, operation, and maintenance manual and with ASME B30.20 "Below-The-Hook Lifting Devices." *** Certification and Documentation of Abatement Required ***
