(a) 3241 N. Clifton, Chicago, Il.: Employees were using wooden job made
ladders without the
filler blocks inserted between the rungs to support the cleats. The
employees were exposed to
a fall hazard when they used the ladder.
Among other methods, one feasible and acceptable method to correct this
hazard is to include,
but not limited to the following:
1) Conform to the American National Standard Institute (ANSI) A14.4-1992,
section 4.5.2
In accordance with 29 CFR 1903.19(c), abatement certification is required
for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
