flat bar were not
marked to indicate the function.

03/02/05, the beam
supporting the 2-ton chain hoist had not been load rated.
Among other methods, one feasible and acceptable method to correct this
hazard is to
ensure that periodic inspections are performed in accordance with ANSI
B30.16-2.1.3-
