40.1-51.1.(A)-Code of Virginia: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: The mixing instructions for the Runway Deicer, Formula Code 1004547, Product Code 004547, were not adequate in that they did not specify chemical addition times or mixing conditions. The improper mixing of the solution resulted in a chemical reaction which resulted in burns to a employee.
