furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees
in that an employee
was exposed to the hazard of being struck by a falling forklift.
a)Warehouse Loading Dock/Bay Door # 1: An employee was using a "Raymond"
forklift truck with a pallet loaded with boxes of paper products which was
elevated 4 feet on the forks and used to ram corrugated boxes into the bed
of a
tractor trailer, on or about 5/13/2005.
ABATEMENT NOTE: The Raymond Owner / Operator Manual provides feasible
methods for
the safe operation of the forklift. Excessive/aggressive
speed/braking/turning may cause the
forklift to tip (page 11). The trailer should be chocked, brakes set and
the jack stands lowered
when the forklift is entering and exiting the trailer to prevent movement
(page 49). The forks
should be no more than 6 inches off the ground unless you are storing or
retrieving a load (page
12).
