a)Production Room:  The hook of the Dayton 2 ton chain hoist
was equipped with a defective safety latch, on or about
8/1/2005.
ABATEMENT NOTE:
Among other methods, one feasible and acceptable method to correct this
hazard is to equip
the hoist with an operable safety latch as described in ANSI B30.16-1993,
Section 16-1.2.9
(hooks). A latch shall be provided to bridge the throat opening of the
hook and retain, under
slack conditions, such items as, but not limited to, slings and chains.
