B56.1-2000 - Safety
Standard for Low Lift and High Lift Trucks, recognize the hazard of
forklift tip-over and the
need for operators to use an operator restraint system, such as a seat
belt:
a)  3020 West Virginia Avenue, Phoenix, AZ:  Employees were operating a
Clark
forklift, model #C500445 and serial #Y355-685-2570, that was lacking a
seat belt.
