40.1-51.1(a) of the Code of Virginia:   The employer, Georges
Chicken,LLC.,  did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause death
or serious physical harm to employees in that a service elevator was being
operated without the barrier gate in
position when the car is moving or not at the floor level(as required by
AMSE A17.1-1990 Section 111.1b).
The Stock Room elevator is operated between the 1st and 2nd floor without
its safety gate closed.  This allows the
stockroom attendant to contact the moving elevator car, and/or enter the
shaft when the elevator is not at his floor level.
One feasible and acceptable abatement method, among others, to correct
this hazard is to equip the elevator with a
mechanical, and/or electrical interlock to prevent the elevator from
leaving without the gate in position.(see AMSE A17.1-1990 Section 111.1b
for locking devices required for "freight elevators".
