Stat. 182.653 subd. 2: The employer did not furnish to each employee, conditions of employment and a place of employment free from recognized hazards which caused or were likely to cause death or serious injury to employees: The platform that was used to elevate employees by a forklift did not have controls that could be elevated with the lifting carriage or forks, and a means of securely attaching the platform to the lifting carriage or forks. This platform was used to perform maintenance at elevation, including changing light bulbs in overhead fixtures.
