Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
Being exposed to fire and/or explosion hazards such as but not limited to
metal parts and debris
from a galvanized metal 10 Gallon Speedaire Paint Tank with lid.
On or about 2/23/05, approximately 11:00am, well stimulation tool
production building, the
employer did not ensure that the pressure vessel (Speedaire 10 Gallon
Paint Tank) under 65 to
70 psi was protected with a protective device such as but not limited to a
safety relief valve.
Among other methods, one feasible and acceptable means to abate this
hazard is to install a
safety relief valve in accordance to ASME standards and the ASME Boiler &
Pressure Vessel
Code, Section VIII, Division 1, 1968ed.
