Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish to employment and a place of employment which was free from
recognized hazards
that are causing or are likely to cause death or serious physical harm in
that employees were
exposed to the hazard of fire/explosion when molten metal contacts liquid
or aerosol
material.
On or about March 9, 2006, at Furnace A of the melt shop, employees were
exposed to the
hazard of fire/explosion while working the furnace, in preparation for
making a tap, when a
hydraulic hose failed causing the furnace to tilt back allowing molten
steel to leak from the
furnace.
Among other methods, feasible and acceptable means to abate this hazard
include but are not
limited to:
1. Use metal armored hoses.
2. Install a canopy/shield over hoses to prevent scrap metal from falling
on the hoses during
charging operations.
3. Ensure that all hoses have been hydrostatically tested prior to
installation.
4. Ensure the furnace slag door seal is suitable for operation and
maintains a good seal to
prevent leaks.
