29 CFR 1910.147(c)(1):  The employer shall establish a program consisting
of energy control
procedures, employee training, and periodic inspections to ensure that
before any employee
performed any servicing or maintenance on a machine or equipment where the
unexpected
energizing, start up or release of stored energy could occur and cause
injury, the machine or
equipment would be isolated, and rendered inoperative in accordance with
29 CFR 1910.147
(c)(4):
a)"A" and "B" elevators at 2160 Madison Ave.
No lock out tag out procedures were provided for each source of energy
(electric
and kinetic) on each elevator for the employee that regularly performed
service
and repairs on the elevators and needed to get to the roof of the "A"
elevator
from the roof of the "B" elevator, on or about 3/21/05.
NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION
FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL
PENALTY OF $1,000.00 AS PER 29 CFR 1903.19.0

energy
(electric and kinetic) on each elevator for the employee that regularly
perform
service and repairs to the elevators and needed to get to the roof of the
"A"
elevator from the roof of the "B" elevator, on or about 3/21/05.

and kinetic) on each elevator for the employee that regularly performed
service
and repairs on the elevators and needed to get to the roof of the "A"
elevator

purpose and function of the energy control program was understood by
employees:
a)"A" and "B" elevators on site at 2160 Madison Ave..
The employer did not conduct training in lock out tag out procedures for
each
source of energy (electric and kinetic), on each elevator, for employees
that
regularly perform service and repairs to the elevators and needed to get
to the
roof of the "A" elevator from the roof of the "B" elevator, on or about
3/21/05.
NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION
