recognized hazards that
were causing or likely to cause death or serious physical harm to

employees were exposed to being crushed by moving elevator counterweight.

Employee placed a part of his body in the runway of an adjacent operational
counterweight while snaking wires for another elevator, on or about
5/24/05.
ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD, AMONG OTHERS, TO
CORRECT THIS HAZARD IS: to follow the employers energy control program in
the
employer`s Safety and Accident Prevention Program, Section 14, Page 1,
where it states that
"Employees will not perform any work on equipment where there is a
potential to come in
contact with energized mechanical or electrical hazards until all sources
of energy have been de-
energized, grounded, or guarded to protect the employees".
and/or
American National Standards Institute (ANSI) Z244.1-93, for personnel
protection,
lockout/tagout of energy sources, minimum safety requirements.
