A.R.S. 23-403.A.: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to their employees, in that the employer did not ensure that a water well drilling rig was inspected and maintained in a safer manner. a) Lot 392, Arvada Rd., Virgin Acres, Scenic, AZ 86432: The employer did not maintain maintenance records on a Foremost dual rotary water well drilling rig, DR12 series, serial number 022270, as specified by the manufacturer. b) Lot 392, Arvada Rd., Virgin Acres, Scenic, AZ 86432: The employer did not designate a competent person to inspect a Foremost dual rotary water well drilling rig, model DR12 series, serial number 022270, before each use. c) Lot 392, Arvada Rd., Virgin Acres, Scenic, AZ 86432: A Foremost dual rotary water well drilling rig, model DR12 series, serial number 022270, was lacking a thorough annual inspection and documentation of those inspections. d) Lot 392, Arvada Rd., Virgin Acres, Scenic, AZ 86432: The employer utilized a swivel hook on a 20' steel drill pipe in lieu of a J-wrench, as recommended by the manufacturer.
