Section 5(a)(1) of the Occupational Safety and Health Act of 1970: Wells Dairy, Inc., 7006 J St., Omaha, NE 68117: The employer did not furnish employment and a place of employment that was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: Employees were exposed to burn hazards from the batching process. On or about 09/20/06, employee(s) connecting pipes from a tank containing hot milk mixture to another pipeline was burned because an open valve allowed liquid as hot as 180 degrees to gush out. Among other methods, feasible and acceptable methods to control the hazards are to: 1. Require disconnect of valve activation hose prior to making connection. 2. Require visual verification of valve position by checking computer panel. 3.Establish, clearly communicate and strictly enforce a work policy that prohibits an employees from connecting the pipe to the main pipeline before that product is ready to be transported. 4. Describe potential hazard in employee training. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.ion
