Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of falling from, and being struck by, the metal
plate connected wood
roof trusses that were subject to movement due to the lack of proper
temporary bracing:
a.  During the installation of metal plate connected wood roof trusses
spanning 38 feet
8 inches, the sheathed gable end and ten trusses collapsed and injured
three (3)
employees on March 1, 2005.  The following factors contributed to the
collapse:
1.  There were high winds on the site at the time of the collapse and the
sheathed
gable end acted like a sail and initiated the collapse due to inadequate
ground to
gable bracing, as specified in Steps 1 through 3 of BCSI B-2.
2.  Bottom chord diagonal bracing was non-existent for the ten (10)
trusses, as
specified in Step 7 of BCSI B-2.
3.  Web member diagonal bracing was non-existent for the ten (10) trusses,
as
specified in Step 6 of BCSI B-2.
One feasible and acceptable method to correct these hazards is to install
the roof trusses in
accordance with the Truss Plate Institute's recommendations which are
contained in BCSI B-1
and B-2 (Building Component Safety Information).  The truss manufacturer
provided a copy of
B-1 when it delivered the trusses to the job site.
