Stat. 182.653 subd. 2: The employer did not furnish to each employee, conditions of employment and a place of employment free from recognized hazards which caused or were likely to cause death or serious injury to employees: On 4/5/05, employee was exposed to fall hazards while working from a personnel basket attached to the forks of an Allis Chalmers #706 forklift elevated approximately 10 feet. The basket was not equipped with a guardrail system. Employee was not wearing personal fall arrest equipment. Location: Mille Ln, Eagle Lake
