Title 40.1-51.1.(a), Code of Virginia: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: (a) Employer exposed employees to struck by/ crushed by hazards, by allowing employee(s) to operate a CAT 277 multi terrain loader with which the back up alarm had been disconnected. This occurred to the rear of 5674 Lierman Circle in Centreville, Va. One feasible and acceptable abatement method, among others to correct this hazard is to follow manufacturers safety operating manual guidelines which state To protect you and others around you, your loader may be equipped with the following safety equipment. Check that each item is securely in place and in operating condition. The back up alarm is included in this list.
