North Carolina General Statute 95-129(1) of the Occupational Safety and
Health Act of North Carolina:  The
employer did not furnish to each of his employees conditions of employment
and a place of employment which
were free from recognized hazards that were causing or likely to cause
death of serious physical harm to
employees:
a)Site -- Facility-Wide:  The employer did not perform a rated load test
of the nineteen bottom-running cranes and hoists installed within the
facility.
REFERENCE:ANSI/ASME B30.11 - Monorails and Underhung Cranes:
Chapter 11-2 - Inspection, Testing, and Maintenance:
Section 11-2.2.2 - Load Test:
a)New, reinstalled, altered, repaired, and modified equipment should be
load tested prior to initial use as determined by a qualified person.
NOTE:Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to have a rated load test performed by a competent person for
each crane or
hoist installed within the facility and to maintain the documentation of
these
load tests on
site in a permanent file.

a) Site -- Facility-Wide:  The employer did not establish an effective
inspection program to ensure
frequent and period inspection by a competent person of the nineteen
underhung cranes and hoists
installed within the facility.
REFERENCE:ANSI/ASME B30.16 - Overhead Hoist (Underhung):
Chapter 16-2 - Inspection, Testing, and Maintenance:
Section 16-2.1.1 - Inspection Classification:
b)1)Frequent Inspection:  Visual examinations by the operator of
other designated person with records not required:
a)Normal Service:  monthly.
b)Heavy Service:  weekly to monthly.
c)Severe Service:  daily to weekly.
2)Periodic Inspection:  Visual inspection by a designated person
who makes records of external conditions to provide the basis
for a continuing evaluation.
a)Normal Service:  yearly.
b)Heavy Service:  semi-annually.
c)Severe Service:  quarterly.
NOTE:Among other methods, one feasible and acceptable method to correct
this hazard is to
develop and  implement an inspection procedure for the cranes and hoists
installed
within the facility that ensures the required inspections covering the
items listed in
B30.16 - Chapter 2 are performed by competent personnel in compliance with
the
schedules contained in the Chapter.

a)  Site -- Facility-Wide:  The rated load capacity for the nineteen
underhung cranes installed within the
facility was not marked such that the load capacity was clearly legible
from the floor.
REFERENCE:ANSI  B30.16 - Overhead Hoists (Underhung):
Chapter 16-1 - Marking, Construction, and Installation:
Section 1.1.1 - Rated Load.  The rated load of the hoist shall be marked
on the
hoist of its load block and shall be legible from the ground or the floor.
NOTE:Among other methods, one feasible and acceptable method to correct
this
hazard is to
clearly mark the rated load capacity of the hoists on the hoist, the load
block, or the
support rails so that the load capacity for each hoist is clearly legible
from the floor.e
